This Site is not for protected health information

The Site is a marketing and informational site. Please do not submit patient information, medical records, or any other protected health information through the Site's forms or by email. If you are a patient with a billing question, contact your practice directly.

• Information you give us. When you contact us or schedule a conversation, we collect what you provide, such as your name, practice name, email address, phone number, specialty, and message. When you subscribe to updates, we collect your email address.
